You can fill one out on tournament day at the Registration table.
You can fill one out when you REGISTER ONLINE.

3. Proof of Age
Bring a drivers license, copy of your birth certificate, or any document that shows your birthdate.
4. Registration Fee
The fee is Php200 per team (Select tournaments vary. Check the "Upcoming Events" page for more info)
You can complete your Registration 2 ways:
Bring your fee and proof of age and you are all set.
